Role Description
This is a full-time on-site role for a Case Manager at Lakeside Manor in Staten Island, NY. The Case Manager will be responsible for coordinating care plans, conducting assessments, collaborating with healthcare providers, advocating for patients, and ensuring quality care delivery.
