Job Description

Position Overview:

We are seeking a licensed Registered Dietitian Nutritionist (RDN) to support our DCN (Developmental Care Network). This role focuses on providing individualized nutrition services, conducting one-on-one patient consultations, and reviewing and signing off on meal plans in compliance with program and clinical requirements.

The ideal candidate has experience working with vulnerable or special needs populations and is passionate about improving health outcomes through nutrition-focused care.

Key Responsibilities:
  • Provide one-on-one nutrition counseling to DCN members enrolled in the 1115 Waiver Program.
  • Develop, review, and sign off on individualized meal plans based on medical, developmental, and social needs.
  • Collaborate with interdisciplinary care teams, including care managers and clinicians, to support holistic care plans.
  • Provide nutrition education to patients, families, and caregivers.
  • Document all patient interactions in compliance with 1115 Waiver and regulatory requirements.
  • Ensure nutrition services align with evidence-based guidelines and DCN program standards.
  • Participate in care coordination activities as needed.
Qualifications:
  • Active Registered Dietitian Nutritionist (RDN) credential.
  • Current state licensure (as required).
  • Experience providing nutrition counseling in clinical, managed care, or community-based settings.
  • Familiarity with special needs populations, developmental conditions, or Medicaid programs preferred.
  • Comfortable working with care management teams and documentation systems.
  • Strong communication and patient engagement skills.
Preferred Experience:
  • Experience with 1115 Waiver programs , DCN, or similar care models.
  • Managed care or value-based care experience.
  • Bilingual skills a plus.
